Copies of your Drivers license, TB test, CPR, and First Aid cards are required
Possession of valid First Aid and Child/Infant/Adult CPR certificates must be issued by a recognized agency.
No online certificates will be accepted.
Please be sure to submit applications by the deadline listed below, or at least one month PRIOR to the start of the sports season, whichever occurs first.
